I have never used the v4 so can't comment on the difference but I think the difference in sharpness is not on the version but rather lens to lens and person to person. A slight misfocus is easy to happen when you are doing mf at f1 on a rangefinder, it really is a hand eye coordination examination when using this lens. Also I think alot of people's noctilux are not really matching the focus with the body being used, I managed to have mine on my m9 spot on after I have adjusted my body a bit (I always wondered why my 50mm and longer lens sometimes would misfocus slightly, turned out the body was back focusing slightly). While the noctilux is not known for sharpness but I find the sharpness I am seeing in my photos well more than acceptable.
